CLICK HERE for a copy of the Medication Authorization Form, to be included with any over the counter and/or prescribed medication sent to school with a student. All medication must come in its original container. Please provide specific instructions on dose and time for medicine to be given. Students may not be in possession of any type of medicine, cough drop, vitamin, etc. that has not been authorized by the nurse.
