T-Shirt Design Approval Form - All shirt designs must be routed for approval by a G-PISD Employee overseeing the project / fundraiser / etc. This could be a Principal, Teacher/Adviser, etc., and the form must be submitted on behalf of a school-sponsored organization by an employee so that an internal contact is available during work hours.
