Attention parents of pre-athletic students and new 7th and 8th grade student athletes:
Attached you will find the current athletic physical form. This form only needs to be filled out by rising 5th graders who signed up for pre-athletics, and students who are new to the district who are in grades 7 and 8. You do not need to fill this form out if you turned in a physical form last year for pre-athletics as a sixth grader or athletics as a seventh grader.
